Accidental death and dismemberment is provided at no cost to all members of Arizona Federal. When a covered injury results in any of the following losses within one year from the date of the accident, Arizona Federal will provide, in one sum, the following amounts (reduced 50% at age 70):

 

 
 
  • Loss of Life - $2,000
  • Loss of Both Hands or Both Feet or Entire Sight of Both Eyes - $2,000
  • Loss of One Hand and One Foot - $2,000
  • Loss of Speech and Hearing in Both Ears - $2,000
  • Loss of Either Hand or Foot and Entire Sight of One Eye - $2,000
  • Loss of Either Hand or Foot - $1,000
  • Loss of Entire sight of One Eye - $1,000
  • Loss of Speech or Hearing in Both Ears - $1,000
  • Loss of Thumb and Index Finger on Same Hand - $500
 
 

Loss means, with regard to hands and feet, actual severance through or above wrist or ankle joints; with regard to sight, speech or hearing, entire and irrevocable loss thereof; with regard to thumb and index finger, actual severance through or above the metacarpophalangeal joints. Injury means bodily injury resulting directly from an accident and independent of all other causes. Loss resulting from sickness or disease, or medical or surgical treatment of a sickness or disease, is not covered. This accident must occur while you are covered under this policy.

 

Other key points:

 
 
  • Member must be at least 18 years of age.
  • Member must complete enrollment form.
  • Coverage will become effective on the first day of the month on the next available quarterly effective date following receipt of your enrollment form.
  • Basic account coverage is for primary member only. Extended coverage can be purchased for joint owners.
  • Coverage is excluded for the following reasons:
    • Suicide or attempt at self-destruction
    • Declared act of war
    • Alcohol intoxication or being under the influence of any narcotic, drug or sedative (unless administered on the advice of a physician)
    • Committing or attempting a felony or illegal act or riding in any aircraft (certified as airworthy) except as a passenger for transportation
